The journey from health visitor to specialist health visitor did not seem too daunting to me when the idea was raised at a team meeting. After all, health visiting practice is already a specialist community role and the infant feeding team had a wealth of knowledge and practical skills for supporting mothers to breastfeed. One morning, sitting in a glasswalled room on an industrial estate somewhere in the south-west of England, the enormity of the task in hand became clear. There was an essay to write, projects to undertake and a multiple-choice exam, as well as practising the skills required to assess infant oromotor function and the delivery of a new specialist service.
